Challenge & Solution
The Problem We Solve
The Challenge
Password-based access is vulnerable to phishing and theft. Hospitals handle sensitive patient records, narcotic prescriptions, and financial approvals. Without second-factor authentication, hospitals face significant data breach risks and regulatory non-compliance (GDPR/HIPAA).
The Solution
The OTP Facility module acts as the gatekeeper for critical actions. It provides a secure, time-based One-Time Password (OTP) engine via SMS, Email, or Mobile App authenticator. It integrates with RBAC to enforce 2FA for high-risk actions like accessing full patient records, approving high-value bills, prescribing controlled substances, or digitally signing discharge summaries.
Key Features
What It Does

Dynamic OTP Generation via Multiple Channels
Generates secure, time-limited numeric/alphanumeric OTPs (valid for 5 minutes). Delivers via SMS (TRAI-compliant routes), Email, or in-app push notification. Support for TOTP (Time-based One-Time Password) algorithm (RFC 6238) for authenticator apps like Google/Microsoft Authenticator.

Contextual 2FA Trigger Configuration
Define custom "Safety Triggers" requiring OTP verification. Default triggers: Login from a new/unrecognized device, Changing Bank Account details, Prescribing a Schedule H/Schedule X drug, Downloading bulk patient data, Approving credit notes > INR 50,000.

Digital Signature & Document Attestation OTP
When a physician finalizes a critical document (Discharge Summary, Medicolegal report, Controlled Substance prescription), they are prompted to enter an OTP. This OTP serves as their legally valid digital signature for non-repudiation, recorded in the audit log.

Patient Identity Verification for Portal Access
For security, patients accessing sensitive parts of the Patient Portal (Report download, Bill payment) are prompted for an OTP sent to their registered mobile number, ensuring the request originates from the actual patient.

Real-Time Audit Logging of OTP Events
Every OTP request, delivery status, and validation attempt is logged in the Security Audit Trail. Tracks which user requested it, the source IP, successful/failed attempts, and the action performed after validation.
Workflow
How It Works
-
Step
1Step 1: User attempts a protected action (e.g., logging in from a new device).
-
Step
2Step 2: The OTP engine intercepts the request, generates a unique OTP, maps it to the user's session, and dispatches it to the user's registered mobile/email.
-
Step
3Step 3: User receives the OTP and enters it into the prompted field in the HIS interface.
-
Step
4Step 4: The engine validates the entered OTP against the generated code and the timer. If it matches and hasn't expired, the engine unlocks the action.
-
Step
5Step 5: The system records a "Verification Success" event in the audit trail and executes the original action (e.g., signature approval, bill payment).

Operator Impact
What Changes for You
Lock-Down Data Breaches
Even if passwords are stolen, data is protected by the second layer of authentication.
Streamline Regulatory Audits
Prove audit compliance with clear logs of who authorized sensitive actions and with what verification.
Enforce Non-Repudiation
Doctors cannot deny signing a document after OTP confirmation, reducing legal liability for fraud.
Empower Secure Remote Work
Clinicians access the system from home via 2FA without compromising security.
FAQ
Frequently Asked Questions
What if a doctor does not have network reception to receive the SMS OTP?
The system supports a fallback to an Authenticator App (TOTP) which generates an offline OTP time-based. Additionally, secret "Backup codes" can be provided.
Can OTP requirements be bypassed in a medical emergency?
Yes. The module has a "Break-Glass" emergency override. If a medical emergency requires immediate action and OTP is unavailable, an Admin can authorize a temporary bypass, which is then logged as a critical security event.
Is the OTP service expensive to run?
Transactional SMS costs are minimal. Integrating with the free WhatsApp Business API or Google Authenticator practically reduces the cost to near-zero for daily 2FA.
